AFFIDAVIT

(for change of name / declaration of one and the same person)

I, Priya Kumari Sinha (formerly known as Priya Kumari), daughter/wife of Mr. Rajesh Sinha, aged about 27 years, resident of Village Bariatu, PS Bariatu, District Ranchi, Jharkhand – 834009, do hereby solemnly affirm and declare as under:

1. That my earlier name was Priya Kumari, which I have changed to Priya Kumari Sinha after my marriage.
2. That my name appears in my various documents as follows:
   1. In my Class 10 certificate: Priya Kumari
   2. In my Aadhaar card: Priya Kumari Sinha
3. That Priya Kumari and Priya Kumari Sinha are the names of one and the same person, i.e. myself.
4. That henceforth I shall be known and identified as Priya Kumari Sinha in all records, documents and dealings.
5. That this change of name has not been made for any unlawful purpose or to deceive anyone.
6. That the facts stated in this affidavit are true and correct to my knowledge.
